The A3 Problem Solving Flow

A3 is not just a paper size; it is a thinking process. Follow these 4 phases to move from a vague problem to a sustained solution.

Phase 01

Define the "Gap"

This is the "Left Side" of the A3. You must clarify the Current State using data, not opinions. Visually demonstrate the gap between where you are and where you need to be.

  • Background: Why does this matter to the business?
  • Current State: Go to Gemba and observe the actual process.
  • Target State: Set a specific goal (e.g., improve from 80% to 95%).
The Rule:

Do not mention solutions yet. If you write "Lack of training" here, you are guessing. Just describe the problem.

Phase 02

Root Cause Analysis

Before fixing anything, you must understand why the gap exists. Use a Fishbone Diagram to brainstorm factors (Man, Machine, Material, Method) and the "5 Whys" to drill down to the root.

  • Fishbone: Cast a wide net to capture all potential causes.
  • 5 Whys: Drill deep. (Why? -> Because X. -> Why X? -> Because Y.)
  • Validation: Can you turn the problem on/off by manipulating this cause?
Warning:

Avoid "Band-Aid" fixes. Addressing symptoms won't stop the problem from coming back next week.

Phase 03

Countermeasures

This begins the "Right Side" of the A3. Now that you know the root cause, propose specific actions to eliminate it. This is your implementation plan.

  • Action Items: What specifically will be done?
  • Owners: Who is responsible for each task?
  • Due Dates: When will it be completed?
Evaluation:

Ensure your countermeasures actually address the root causes found in Phase 2. If they don't link up, the plan is flawed.

Phase 04

Check & Sustain

Did the plan work? Compare your new data against the targets set in Phase 1. If you succeeded, you must standardize the new process to prevent backsliding.

  • Check Results: Measure the process again.
  • Standardize: Update SOPs, retrain staff, and share knowledge.
  • PDCA: If the target wasn't met, Plan-Do-Check-Act again.
Standard Work:

Without a new standard, the process will revert to the old way within 30 days.

Why is it called an "A3"?

When should I use an A3 vs. an 8D Report?

Can I just jump to the Solution box?

No. The magic of the A3 is in the "Left Side" (Analysis). You must strictly define the Current State and verify the Root Cause before you move to the "Right Side" (Countermeasures). Skipping the analysis phase is the most common reason improvement projects fail.
