The 5S Audit Process

Conducting an audit is not just about checking boxes; it is a cycle of observation, measurement, and improvement. Follow these 4 phases to maintain a world-class workplace.

Phase 01

Walk the Gemba

Take your tablet or clipboard and walk the actual floor. Do not rely on memory or photos sent by others. You must see the current condition with your own eyes.

  • Observation: Look for clutter, safety hazards, and missing labels.
  • Interview: Ask operators if they have what they need.
  • Respect: The goal is to help, not to catch people doing wrong.
Focus Area:

Check "hidden" areas: under desks, behind machines, and inside drawers. Entropy hides where you don't look.

Phase 02

Score the 5 Pillars

Rate each of the 5S categories (Sort, Set, Shine, Standardize, Sustain) on a scale of 0-5. Be objective. A score of "5" means the standard is world-class and self-sustaining.

  • Sort: Is only needed equipment present?
  • Set in Order: Is there a place for everything?
  • Shine: Is the area inspection-ready clean?

Assign Corrective Actions

A low score is meaningless without a plan to fix it. For every non-conformance identified, create a specific action item with an owner and a deadline.

  • Owner: "Maintenance Lead."

Close the Loop:

The audit is not "complete" until the action items are verified as closed in the next cycle.

Visualize Trends

Post the audit results on your Visual Management board. Showing the trend line (Audit Score vs. Time) keeps the team motivated and accountable.

  • Transparency: Everyone should know the score.
  • Competition: Friendly rivalry between shifts can drive scores up.
  • Celebration: Recognize teams that consistently score above 4.0.

Common Questions

How often should we conduct a 5S audit?

Frequency depends on maturity. New 5S zones should be audited weekly to build habits. Mature zones can move to monthly audits. However, the most effective audits are "layered"—supervisors check daily, managers weekly, and executives monthly.

What is a "Red Tag" Area?

This is a temporary holding zone created during the Sort (Seiri) phase. If you are unsure if an item is needed, place a red tag on it and move it to this area. If no one uses it after 30 days, it is discarded or sold. This prevents employees from hoarding "just in case" items.

How is the score calculated?

This template uses a weighted average. You rate various criteria (e.g., "Floors are free of debris") on a scale of 0 to 5.

0 = No Standard
3 = Standard Defined but inconsistent
5 = World Class (Sustained automatically)

The tool then averages these to give you a total percentage score for the zone.

Who should perform the audit?

Ideally, audits should be cross-functional. Having someone from "Office Admin" audit the "Warehouse" provides fresh eyes and prevents bias ("Oh, that pile of junk is always there"). It also reinforces that 5S is a company-wide standard, not just a manufacturing rule.
